"For students who travel via school bus, families must complete the screening, including checking their temperature, prior to boarding the school bus to make certain that their child is well enough to ride on the bus and attend school. Bus personnel will not be conducting screenings of children to prior to boarding the bus. School site designated staff will be checking for completion of student health screening at entry. Students who lack the screening clearance will need to be supported in completing this screening process at the school, particularly young children.
For morning student drop off, every effort should be made to ensure that student health screenings are completed prior to families dropping off their children to school, especially for our younger students."
Paper Copy of Health Screening Questionnaire:
"If students are not able to pre-screen using the online tool, schools must provide families with several copies of the health screening questionnaire so families can complete the questionnaire at home."
In-Person Health Screening Tool:
"Any person who needs to complete the health screening in-person will undergo a temperature check.

Schools should determine the best way to implement on-site screening for students using either the online health screening tool or paper copies of the screening questionnaire. Young children may require additional support from a staff member."
If NYCDOE families are unable to pre-screen their child(ren) at home using either the online tool or paper questionnaire, families will need to complete their child’s screening in-person at the school building prior to their child’s entry.
Important Updates on In-Person Learning Start Dates
Remote Learning (100% Online):
Students who are enrolled in fully remote learning (Cohort C) will still begin full-day instruction on Monday, September 21.
Blended Learning (in-Person and Remote)
Students in blended learning will be learning remotely on Monday, September 21 until their in-person start date as outlined below.
Students in Cohort A will beginin-Person learning on Monday, September 21 at 8:30am.
Students in Cohort B will begin in-Person learning on Tuesday, September 22 at 8:30am.
Kindergarten - Fifth Grade students in Cohort A will begin in-Person learning on Wednesday, September 30 at 8:30am.
Kindergarten - Fifth Grade students in Cohort B will begin in-Person learning on Tuesday, September 29 at 8:30am.
Arrival and Dismissal Information
New School Time: 8:30AM- 2:00PM
Students taking part in Blended Learning should arrive to PS 12 (430 Howard Avenue) at 8:30AM. Students will be dismissed at 2:00PM.
Please see the attached image for information regarding which days your child will be attending school in person.
The first day of Blended Learning (in person) will be Monday, September 21, 2020.

2020-2021 School Year: Important Dates
Wednesday, September 16, Thursday, September 17, and Friday September 18, 2020
All students will reconnect to their school community. Students will learn the health and safety procedures. Technical connectivity will also be checked. Students will have wellness checks with their teachers and guidance staff.
Monday, September 21, 2020
All schools will begin full-day teaching and learning.
Cohort A and Cohort B - September 21, 2020
Students in blended learning (combination of in-person and remote) will begin to report to buildings, depending on their in-person schedule.
Cohort C - September 21, 2020
Students in 100% remote learning will all begin full-day learning.
PS 12 Cohorts and School Plan
For the upcoming school year, P.S. 12 students will be assigned into a cohort. This cohort will indicate the specific days of the week that your child will receive in person learning and/or remote learning.
Cohort A will receive in-school instruction on Monday and Wednesday. On Tuesday and Thursday they will receive remote instruction. Fridays will be dedicated to academic intervention for students with a demonstrated need.
Cohort B will receive in-school instruction on Tuesday and Thursday. On Monday and Wednesday they will receive remote instruction. Fridays will be dedicated to academic intervention for students with a demonstrated need.
Cohort C will receive remote instruction (100% online) Monday through Friday.
